In developing countries, such as those in South Asia and Africa, conjugate vaccines have the potential to significantly alter the disease burden associated with the aetiology of middle ear infections, thus altering the management of these diseases. To determine whether the administration of conjugate vaccines to children has an impact on the aetiology of otitis media (OM). Cochrane Library, Medline, Embase, Database of Abstracts of Reviews of Effectiveness and National Health Service Economic Evaluation Database. Trials published until July 2009 were eligible for inclusion. Inclusion criteria were randomised controlled trials with a minimum of 6 months follow up. Studies were assessed using the GRADE approach. From 200 studies screened, nine were included in the review. One study examined the impact of conjugate vaccine on the incidence of OM (CPS-OM: 85% placebo). A further study compared the prevalence of OME in children aged 4-10 years before and after the introduction of the 7-valent pneumococcal conjugate vaccine (PCV7). Another study reported a comparison of the prevalence of OM in children (aged 1-5 years) between countries using either PCV7 or no PCV. The results of the first three studies were of low quality and those of the latter two studies were of moderate quality. Two studies found a decrease in the prevalence of OM and one study found a decrease in the incidence of OM following PCV7 introduction. The fifth study found that children in PCV7 vaccinated areas had a lower prevalence of OM compared to children in areas that did not use PCV7. The remaining studies found no difference in the prevalence or incidence of OM in either countries that used PCV7 or countries that did not use it. Only a small number of studies were found to evaluate the impact of conjugate vaccines on the aetiology of OM. The majority of these studies were of poor quality and the results were inconclusive. However, some studies that evaluated the impact of PCV7 suggest that a reduction in OM may be seen.
