We reviewed our series of five patients with intra-articular ligament injury of the knee in order to describe the peculiarities of this lesion, to suggest a definite diagnosis and to establish appropriate treatment. All of our patients presented a direct flexion-adduction-internal rotation (D.F.A.I.R.) mechanism in association with a transverse tibial shaft fracture. The knee was valgus in two cases and varus in three. The injury of the posterior cruciate ligament (PCL) was at the midsubstance level in two cases, partial tear in two cases and complete avulsion in one case. The isolated injury of the lateral collateral ligament was at the midsubstance in one case and partial tear in the other four. The medial collateral ligament injury was at the midsubstance in three cases. Only two patients had a continuous distal tibial fracture. The displacement of the distal tibia fracture was rarely associated with the injury of the PCL. The treatment of these injuries was based on reconstruction with double bundle techniques. With a mean follow-up of 24 months, our results were satisfactory in every case.Q:
